Despite advances in the prevention and treatment of HIV infection, the burden of new infections and disease remains high, especially in Africa. So as to meet agreed international targets, improvements are needed in population-wide HIV testing, provision of care and achieving viral suppression. Tailoring provision to local settings is particularly important to optimize retention in and effectiveness of care. In a Policy Forum, Peter Ehrenkranz and co-authors present a cyclical cascade of care for people with HIV infection, aiming to facilitate assessment of outcomes. This in turn should assist policymakers, health workers and others to identify and overcome barriers in care systems and reduce progression to advanced disease.
