Please indicate if the patient has any significant past medical history, in particular immunosuppression that might have influenced the initial presentation of multiple lesions or the subsequent course of recurrent sterile inflammatory lesions. Was additional immunosuppression screen performed?

Reviewer #1: I think that this is a well written case report and adds to the spectrum of paradoxical reactions seen in BU. I think it is of significant interest to clinicians treating BU, with practical implications, and also to those who manage other mycobacterial infections where paradoxical reactions are problematic.

Reviewer #2: This is a clearly written well documented case of a late paradoxical reaction in an Australian patient with Buruli ulcer. The histological image is particularly useful. The case is worth presenting because while paradoxical reactions are well documented most clinicians would be concerned about a re-infection this many years after the original diagnosis.

Reviewer #2: I think a clinical photograph of the late presentation could be a useful addition.

Were there any other cultures that were positive (eg for S. aureus)?

The original diagnostic specimen was PCR positive but "not cultured". The reference laboratory that performs the PCRs in Victoria routinely cultures all first PCR requests so may have this information if asked. If they do, it would be worth reporting. However I have no doubt the original diagnosis based on PCR alone is correct in this clinical setting.
