By contrast, a negative QALY difference is represented on the left-hand side. The Y-axis shows the cost differential. If the intervention is associated with a cost reduction, the cost differential is negative (lower part of the graph), while a positive cost differential (upper part of the graph) indicates an increase in costs due to the intervention.
