Reticular opacities in the base of both lower lobes were noted, suggesting interstitial pneumonitis (B) CT image at the level of carina demonstrates multiple round low attenuating areas in both lungs (arrow), suggesting emphysema. He underwent right lower lobectomy and systemic nodal dissection via video assisted thoracoscopic surgery. During the operation, there was severe pleural adhesion requiring nine hours for adhesiolysis.
