Amendment to acknowledgements

I cannot apologise enough for the errors I have made in the acknowledgment section of this paper and would like people to be aware of the following corrections:

I would like to state that the acknowledgment to the European cystic fibrosis society steering group should have been:
"We would like to thank the European Cystic Fibrosis Society Patient Registry for providing patient data and thank the individual country representative for allowing the use of their data (www.ecfs.eu/projects/ecfs...)"

In addition the acknowledgment to V.Gulmans for Germany 2011 data should be corrected to the following:
"Martin Stern for the Germany CF 2011 data"

Once again I am sorry for any problems this has caused and unfortunately I was unable to get these corrections re-published.
