I read with great interest the article by Dai et al addressing Nomograms for Predicting the Prognostic Value of Pre-Therapeutic CA15-3 and CEA Serum Levels in TNBC Patients.Multivariate analysis suggested that pre-therapeutic CA15-3 and CEA levels are independent predictive elements for OS (p = 0.022 and p = 0.040, respectively) and DFS (p = 0.023 and p = 0.028, respectively). In discussion part, they did not mention a probable association between subclasses of TNBC and serum CA 15-3 and CEA levels. As we know that Cluster analysis identified 6 TNBC subtypes displaying unique GE and ontologies, including 2 basal-like (BL1 and BL2), an immunomodulatory (IM), a mesenchymal (M), a mesenchymal stem–like (MSL), and a luminal androgen receptor (LAR) subtype described by Lehmann and their colleagues. These subclasses have different pattern of pathology and survival differences, and different responses to systemic treatment. More differentiated ones are expected to secrete more CA 15-3 and CEA and and give less response to systemic chemotherapy
