Competing Interests
JL has received fees from GlaxoSmithKline as a member of an Independent Data and Safety Monitoring Committee for a trial of the efficacy of vaccination against HSV. Professor Julian Little was an academic editor for the journal at the time the paper was submitted but does not hold this position now.
