Who should pay for costs associated with choosing to not have two perfectly healthy legs?

Knowledge of and respect for the desires of BIID individuals are the first steps in providing care and may decrease the huge burden they experience." And who do the authors suggest bear the financial burdens associated with the amputation of, say a perfectly healthy leg?
