A disadvantage of natural ventilation is the difficulty in controlling direction of airflow due to the absence of negative pressure.

Especially in large cities in resource poor contexts, outdoor air quality is very poor. The highest levels of ozone and of PM2.5 and PM10 are reported by the World Health Organization as being in the large cities throughout the developing world. Increasing outdoor air ventilation rates means creating an indoor air atmosphere that is as contaminated as the outdoor air and may compromise the immune response of patients already at risk.
