Diabetes is a worldwide scourge that is growing substantially owing to unfavorable trends in diet and physical activity, exerting a toll of morbidity and mortality that is especially burdensome in low-income and middle-income countries. Preventing diabetes through individual-level and policy approaches is therefore an urgent priority that is the focus of PLOS Medicine's Special Issue on Diabetes Prevention. In an Editorial to conclude the issue, Guest Editors Nicholas Wareham and William Herman draw together themes that have been addressed by research studies and discussion pieces in the issue. Wareham and Herman discuss some of the challenges for researchers and policy makers in developing effective and equitable solutions to the worldwide problem of diabetes, and call for much greater investment in gathering evidence to underpin diabetes prevention efforts in developing countries.
