All CVD includes ICD-10-AM diagnosis codes I00–I99, G45, and G46; ischaemic heart disease includes I20–I25; peripheral vascular disease (PVD) includes I70–I74; stroke includes I60–I69, G45, and G46; heart failure is I50; and other CVD is all CVDs except those listed above. Vertical lines indicate 95% CIs.
