How is a deficiency state determined

This implies that underweight people would have higher apparent levels of Vitamin D. But could this "hide" a deficiency? Is there any actual evidence of a physiological deficit among either obese people, or those with vitamin D-related SNPs who normally have lower 25(OH)D concentrations?
