PubMed has an important new feature which they have kept rather quiet about - outputting search results as RSS 2.0 feeds.
Look under the "Send To" pull-down menu on the PubMed search results screen, and you will see "RSS feed". I have tried it out using the "My Yahoo" RSS newsreader page.

This RSS feed could function in a similar way to email notification of new articles relevant to an old search, but potentially offers more exciting functionality e.g. continuously updated references section for a medical article on a web page.
